MHealth Fairview Longitudinal Interprofessional Integrated Clerkship (FLIIC)

Considering FLIIC? 

The MHealth Fairview Longitudinal Interprofessional Integrated Clerkship (FLIIC) is a nine-month interdisciplinary educational experience in which medical students train in a community setting. FLIIC students are primarily hosted by the MHealth Fairview Eagan Clinic and MHealth Fairview Ridges Hospital. Our students work directly with physician preceptors and also spend time learning from interdisciplinary colleagues (pharmacists, pediatric rehab, physical therapy, diabetes educators, etc). Working in a community setting, students have the opportunity to follow patients longitudinally across different phases of care.

Our program has three core pillars: community medicine, personal leadership development, and interdisciplinary care for patients. 

FLICC is a great match for independent, self-directed, adaptable students with an interest in longitudinal community medicine. Each year our students have forged new educational opportunities with their creativity and independent thinking.

MHealth Fairview Ridges Hospital is a growing 165 bed 5-star CMS rated community hospital with the 5th busiest ER in the state by volume (70K+ annual visits). MD/APP specialties include Anesthesia, Critical Care, Cardiology, Colorectal Surgery, Emergency Medicine, ENT, General Surgery, Hospitalist Medicine, Nephrology, Neurology, Neurosurgery, Oncology, Palliative Care, Pulmonology and Urology. There are very few trainees at this hospital outside of FLIIC creating an “unopposed” learning environment in which students have outstanding opportunities to be hands-on and an integral part of the team. Ridges has proven to be a very welcoming place for medical students with an extremely collegial environment.

MHealth Fairview Eagan Clinic is a busy outpatient primary care clinic. There are nearly 20 outpatient providers, the majority of whom are dual boarded Internal Medicine-Pediatrics physicians. This particular clinic has a longstanding commitment to education and is a residency continuity clinic site for the University of Minnesota Internal Medicine-Pediatrics residency program. Housed within our clinic building are several other specialties which care for our patients and offer opportunities for teaching including Urgent Care, Pharmacy, Physical Therapy, Medication Therapy Management, Diabetes Education, Pediatric Rehabilitation, Mammography/Dexa, etc.

FLIIC students integrate into one of our 4 stations and work directly with 2-3 primary preceptors. Over 9 months FLIIC students get to know the staff supporting patients and have the opportunity to work directly with other disciplines.

Advantages of FLIIC

  • FLIIC students work directly with preceptors in a community setting as unopposed learners (very few residents/fellows). Students are first up for procedures and patients with excellent learning opportunities.
  • The community setting offers a great environment to amass a critical amount of medical knowledge, and also offers higher acuity opportunities as students grow. For example, later in the year students spend some of their internal medicine time in the ICU.
  • As a small program our students get individualized attention from the co-directors. We meet weekly as a program for check-ins and individually in a more formal capacity to ensure that students are feeling supported, progressing academically, and have the experiences they need for career discernment. 
  • We value students as learners but also as people who have lives outside of medicine. We offer 10 days of PTO with a good amount of flexibility to ensure that students have time outside of medical school as well.

Community Medicine

FLIIC offers students the unique opportunity to train in a community hospital setting, while still seeing a diverse patient population and extremely broad scope of pathology typically found in larger academic centers. The Ridges ER is the fifth busiest emergency department by volume in the state of Minnesota, and with few trainees around students get the sort of hands-on experiences and longitudinal interpersonal relationships often reserved for smaller hospital/clinic sites.

Leadership

FLIIC encourages students to explore personal aspects of leadership - this does not mean grooming the next CMO (though it can!) but our students are asked to consider what they bring to the table when interacting with others. Every Thursday, FLIIC medical students are joined by students from several other health professional schools (Physical Therapy, Doctor of Nursing Practice, Pharmacy, Masters in Health Administration, Masters in Public Health, etc) to explore the personal aspects of leadership as they impact the clinical experience. The curriculum is facilitated by Tom Gilliam, RN, former Chief of Staff of the Medical School and the co-founder of several leadership programs for resident and attending staff physicians. Interdisciplinary Care

Much of the care of a patient occurs outside the walls of our hospitals and clinics and is delivered by a team greater than just physicians. FLIICs Leadership Seminar offers the opportunity to learn more about other teams involved in patient care and their journey to their role. In clinical practice, FLIIC offers the opportunity to learn directly from other disciplines. In the past we’ve had students learn how to apply/wear continuous glucose monitors with Diabetic Educators, observe the process of mammography/dexa, and even follow a child with developmental delay as they work with Speech, Physical Therapy, and Occupational Therapy. This time is valuable in several ways including understanding what other teams do and how they get there, ability to counsel patients about what comes next when you put in a referral, and expanded knowledge from experts- for example, Diabetes Educators know more about treating diabetes with nutrition than a physician will.

Clerkships

FLIIC starts slightly later than other longitudinal integrated clerkships, aligning with other health profession schools, and covers components of both the clinical immersion phase and specialty specific clerkships. Students complete select block clerkships in the traditional model before entering FLIIC and are considered advanced immersive students upon starting

The educational schedule is intentionally designed with a great deal of flexibility to help promote independent learning and the ability to follow patients across the system. Because FLIIC is a smaller program, students have the ability to tailor the experience to their individual learning objectives. We depend on our students for ongoing quality improvement projects and development of new clinical experiences.

  • In all clerkships students work directly with attending preceptors. This allows students to be first in line for procedures and for students to see patients who advance their educational goals.
  • Clerkship scheduling and timing is thoughtfully approached so that a critical mass of knowledge builds and allows students to thrive. 
    • Some clerkships benefit from immediate continuity such as Internal Medicine and General Surgery. These clerkships are scheduled as one week bursts so that students can follow the same patients throughout the week.
    • Other clerkships are threaded throughout the year so that students can see the evolution of care. For example, students are assigned to primary preceptors in OB/Gyn and primary care and work with them over the course of the year, allowing entrustable relationships to develop over time. 
    • Emergency Medicine is placed in the middle of the year, once students have a good foundation of medical and institutional knowledge. Students complete 2, two-week bursts of emergency medicine in one of the top five busiest emergency departments in the state. Students see a high volume of patients and perform a variety of procedures with an appropriate level of supervision.
  • In the later third of the program, FLIIC students work with the co-directors to create their own career discernment elective. In the past students have spent time advancing skills in PM&R/Neurology/Sports Medicine/Rehab, explored advanced surgical subspecialties, and worked with a potpourri of medical subspecialties. 

Required Clerkship Credits Completed Outside of FLIIC

FLIIC is a biphasic program, covering clinical immersion and specialty specific clerkships. We start slightly later in the year in order to align with the calendars of other health professions schools. The majority of other required clerkships are expected to be completed prior to starting FLIIC. As an advanced LIC we incorporate clerkships such as Emergency Medicine, the outpatient immersive experience, and our FLIIC Elective Plus (an opportunity to design your own career discernment elective). As unopposed learners the sky's the limit for learning and our goal is that you leave FLIIC acting more as a later 4th year student.
